Mr. Peter Dawsin comes from his PCP office with a prescription for Bactrim DS Q12H for 3 days for urinary tract infection.  Mr. Dawsin has documented allergy to co-trimoxazole. It is not safe to dispense this medication. In addition, patient is taking warfarin for atrial fibrillation that can interact with Bactrim.
