A patient approaches the counter with a burning sensation in his chest, wondering if OTC therapy is appropriate due to his other current conditions.

“I’ve been taking my Ibuprofen as needed- usually about 4 times a day for my arthritis pain.”

This patient is not a candidate for just an OTC recommendation. He needs to see his PCP because of his chronic Ibuprofen use. This could be the underlying factor for his symptoms, causing a stomach ulcer or something more serious than an OTC recommendation.
